Access Your Health Information

Many members of the UW Medicine Accountable Care Network offer online resources to help you view and manage aspects of your healthcare. You can review your clinic or hospital visit information, see your test results, communicate with your physicians and healthcare professionals through secure-messaging, review and renew prescriptions, manage your appointments, see your hospital discharge summary, and access copies of your medical records. These online resources will vary depending on where you receive care.

To access online services, please select the organization that provided your care below.


Use Capital Medical My Health Record to access your health records and lab results, review your medications and hospital discharge summaries and communicate with your providers.

Login to MultiCare Connected Care MyChart to schedule doctor’s appointments, request prescription refills and review your health history.


Use Overlake Hospital ONE Chart to communicate with your doctor, access your medical records and lab test results, renew prescriptions and manage your appointments.


Complete an online form to request a copy of your medical records on the Seattle Cancer Care Alliance website.

Login to MySeattleChildren’s to send a message to your Seattle Children’s provider, check lab and radiology results, view medication lists, clinic visit summaries, and discharge instructions, and view upcoming scheduled appointments.


Login to the Skagit Valley Hospital Patient Portal or the Skagit Valley Clinics MyEChart portal to manage your health online.
