Access Your Health Information

Many members of the UW Medicine Accountable Care Network offer online resources to help you manage aspects of your healthcare — such as viewing your visit information, seeing your test results, communicating with your providers through secure-messaging, reviewing and renewing prescriptions, managing your appointments, seeing your hospital discharge summary, and accessing copies of your medical records. The resources available will vary depending on where you received care.

Use Capital Medical My Health Record to access your health records and lab results, review your medications and hospital discharge summaries and communicate with your providers.

Login to myIslandHealth to access your personal health records, test results and billing activity.


Login to MultiCare Connected Care MyChart to schedule doctor’s appointments, request prescription refills and review your health history.


Login to the PeaceHealth PatientConnection to communicate with your doctor, access your test results, renew prescriptions and manage your appointments.


Complete an online form to request a copy of your medical records on the Seattle Cancer Care Alliance website.

Login to MySeattleChildren’s to send a message to your Seattle Children’s provider, check lab and radiology results, view medication lists, clinic visit summaries, and discharge instructions, and view upcoming scheduled appointments.


Login to the Skagit Valley Hospital Patient Portal or the Skagit Valley Clinics MyEChart portal to manage your health online.
